The Diaper Bank office must receive this form NO LATER than 10:00 a.m. on the Friday before your pick-up date. Orders not received by the deadline cannot be filled.

 

Pick-up time from Beacon is Thursdays between 9:30-11:30 a.m.

 

Beacon is located at 308 W. Glenn. During warehouse construction, use the main entrance at the Glenn parking lot.
